Reservation Instructions

1. Check calendar on MVBA website to determine whether desired dates are available.

2. Fill out the Block Party Request Form located on the MVBA website (this requires checking which items will be used) and forward it to Chuck Cotton by email at pastorchuck@calvarybaptistmiddletown.org.  Note: should you need to contact by phone, the number is (937) 302-6697.

3. Wait for confirmation by telephone or email.

4. Once confirmation is received, check calendar to verify dates are locked in.

 

5. The Block Party Trailer will reside at Calvary Baptist Church, 1601 Jackson Lane, Middletown, OH 45044. Pick up BPT no earlier than 24 hours in advance of the reservation date.

6. After usage, retrieve the Block Party Check-In form from the MVBA website, fill out and e-mail to pastorchuck@calvarybaptistmiddletown.org. FAILURE to return this form will result in a $25 charge for the church failing to fulfill this obligation!

7. Return the BPT to Calvary Baptist within 24 hours of the reservation date.

8. If any item is broken or missing, please notify the MVBA Ministry Assistant immediately at (937) 302-6697. If items are reported missing, a reference will be made to required checklists. Missing items will then become the financial responsibility of the last church using the equipment. Cost for repair of equipment broken during normal use will be the responsibility of the association ASSUMING that the malfunction is reported immediately. Otherwise, if another church discovers the broken equipment, the last church using the equipment will be billed for the repair. So…simply report the problem if the equipment is broken while being used for your function!

9. In order to insure that you will be serving safe and non-contaminated food, please clean the equipment before using it.

10. Photographs posted inside the trailer make it easy to correctly reload.

**Note. The cotton candy machine is continuously returned dirty. The directions for proper cleaning are within the trailer (where the clipboard is) inside an envelope marked "Cotton Candy Machine". Please read them carefully and gently clean the machine.

11. Make check or money order payable to MVBA and send to MVBA, c/o Diana Cotton, 7714 Hunters Trail, West Chester, OH 45069
